Psychosocial assessment

A psychosocial assessment includes assessment of patient’s social, emotional and environmental circumstances. It enables the medical social worker to assess the impact of illness on a patient or his/her family and plan appropriate support.

Professional counselling for anyone whose social/emotional problems may be of concern to themselves or to their families in relation to their admission for orthopaedic rehabilitation.

Issues addressed such as:

Life changes - Impacting on health, homelessness, relationship difficulties, RTAs/trauma, elder abuse, ageing, loss of independence and bereavement.

Information and advice - In relation to social welfare benefits and entitlements, community supports and the hospital system etc.

Discharge planning - Medical social workers facilitate discharge as part of the multidisciplinary team. We liaise with members of the community, both statutory and voluntary, to arrange services for patients and engage in a problem solving process to ensure that the discharge is appropriate and in the best interests of the patient and family and that all possible help and support has been considered.
